SURVEY SPORTS SETUP PROCESS |

| 1. On Home Page you will see the packages available, and the descriptions of each one. |
| 2. Choose either "Sign In" (if you are already registered) or "Sign Up" (if you are a new user). You will then be sent an email containing a link to proceed to the payment options. This is for verification purposes. |
| 3. Upon receipt of the confirmation email, click on the link to start the payment process. You will be required to submit the method of payment (Visa, Master Card, etc). Upon completion, you will receive an authentication email to act as a receipt of your payment, and then you will be transferred to the admin area where you can name your organization and build your survey. |
| 4. In the admin section, you will be able to see everything (including all comments, suggestions, and answers to each question). If there are other people in your organization and you want them to be able to see everything, you need to add them as a new admin by adding their email address into the admin section. |
5. Once you have completed building your survey, SurveySports.com creates a new web page for you and your organization to promote the survey and keep your members updated. You can copy and paste the link (ie. www.surveysports.com/YOURORGANIZATIONNAME) onto your website, flyers, or any media you choose. Another way to find the survey is to go to www.surveysports.com and select your organization's name from the drop-down at the top/right corner of our pages. Your survey will stay active for up to six months. At that time, all results will be removed from the SurveySports.com database.
**Please make sure to log in and review or print the results prior to the six-month period (as they will be deleted permanently). For"Basic" users: you have completed the setup process. For "Intermediate" users: please continue. |

| 6. The next step for you is to select the questions you want on your survey. In this area you'll have a wide variety of survey questions to pick from and then you will be able to review your survey before you make it final. You will enter the name of your organization, be able to customize the site with your logo, and make sure you're satisfied with the questions you select before making the survey final. You can re-do the questions you want to ask as many times as you like until you click, "submit." Once "submit" is clicked, the survey CAN NOT be changed. If you chose "Intermediate," you have completed the setup process. For "Advanced" users: please continue. |

| 7. For users that chose, "Advanced," you will be sent to our "Contact Us" page, where you will set up specific and direct communication with us, so that we can completely customize your organization's Surveys. There are no limits (other than those you set) to how tailored, how detailed, and how complete your Surveys can be. Once we have received all of the questions and details of your custom survey, Survey Sports designers will provide you with a cost to complete the project. From that point, when you approve of the proposed survey, and payment is received (in advance), you can expect a minimum of 10 business days before the survey will be available for you (and your participants) to begin filling it out. |
