Should you have problems with the download, please contact Survey Sports Support at 330-288-8789 or email your request or for additional assistance at support@surveysports.com. Survey Sports Tech Support representatives must be made aware of any problems. If the issue can not be resolved and Survey Sports Technicians are unable to complete the transmission, you may request a credit to your account for your electronically downloaded survey. Please contact Survey Sports Support at 330-288-8789 for refund requests. Refund requests must be received within 15 calendar days of the purchase date.
